Dogwood Valley Puppies for the purchase of a puppy.
Health Guarantee & Purchase Agreement
for
Puppy ID#: Breed: Color:
Sex: Date of Birth: Registration:
* This guarantee is non-transferable and applies only to the original buyer of the puppy.
* This is a contract between Dogwood Valley Puppies and the buyer of the afore-mentioned puppy. To the best of Dogwood Valley Puppies' knowledge, this dog is in good health at the time of delivery. There are no cash refunds, for any reason, after the puppy has left our custody. Replacement of the puppy, if necessary, will be made with the next available puppy of the same breed, sex, color and of equal value, provided the following requirements are met:
The puppy must be declared by a licensed veterinarian to be in a life-threatening condition, caused by a contagious disease, within 72 hours (three days) of the time the puppy leaves our custody and a written report from the same licensed veterinarian must be submitted to Dogwood Valley Puppies.
If the puppy dies, you must have followed all advice provided by your licensed veterinarian and have an autopsy performed by your state lab and submit a copy of that report to Dogwood Valley Puppies.
Because all of the puppy's immunizations are current, a false-positive 'CITE' or 'SNAP' test for Parvo Virus is possible. As a result, only a positive culture or growth test for Parvo Virus, performed by a licensed veterinarian or state lab, will be accepted.
Dogwood Valley Puppies is responsible for the following congenital defects for a period of six (6) months after the date of purchase: hip dysplasia, deafness, demodectic mange, patella luxation, heart murmur, portosystemic shunt or blindness. If the buyer is told of any congenital defect, and it is added to the contract, then Dogwood Valley Puppies is not responsible for that defect. In the event any of the preceding defects are present, Dogwood Valley Puppies will replace the puppy following the previously stated guidelines.
* Dogwood Valley Puppies does not guarantee against hypoglycemia or any other common problems such as; umbilical (belly button) hernias, diarrhea, Coccidia, Giardia, worms, mites, or any other common internal or external parasite.
* Dogwood Valley Puppies makes no guarantee as to disposition, conformation, size or fertility. All puppies are sold as pets only.
* Dogwood Valley Puppies, or any affiliate or associate thereof, will not be responsible for any shipping expenses, boarding expenses or veterinary bills incurred after the sale has been completed and the puppy has left our custody.
* Dogwood Valley Puppies is not responsible for any accidents or travel delays that occur after the puppy has left our custody.
* Dogwood Valley Puppies makes no guarantee as to the availability of flights or flight schedules and reserves the right to cancel or re-schedule a flight at any time, without notice, if necessary to protect the health and safety of the puppy.
* Registration papers, if applicable, for your puppy, will be mailed to you within four (4) weeks from the date the puppy leaves our custody.
* If, for ANY reason, Dogwood Valley Puppies deems the puppy unfit for travel or purchase, any and all money paid for the puppy will be immediately refunded, or a replacement puppy will be provided at the Buyer's request.
